Services are by appointment only. Please call us at 812-689-5751 to schedule with our staff.
We bill with the following insurance companies: Anthem BC/BS, Cigna, UnitedHealthcare, Medicaid, Tricare East, Sagamore, SIHO, and GPA. If you do not have insurance, there will be no charge for vaccinations. We are happy to serve you and your family!

Additional Services
- Drug Tests: Free kits are provided by Margaret Mary Hospital. Pickup is in the Health Department lobby.
- HIV Tests: Free tests are given at the Health Department the first Thursday of every month. Testing is free and confidential, and no appointment is necessary. Testing is done through Aspire Indiana Health.
- Vaccine Clinics: Walk-in clinics are held in our office every Thursday from 8am-5pm and every third Saturday from 9am-12pm.
Indiana School Immunizations
Parents can get immunization records from our office or from their health care provider. They can also use the secure MyVaxIndiana website to print their child’s official immunization record from home by using a personal identification number (PIN).
